How It Works

CaO2 = (1.34 × Hb × SaO2) + (0.003 × PaO2). The first term represents oxygen bound to haemoglobin (each gram of haemoglobin carries 1.34 mL O2 at 100% saturation). The second term represents dissolved oxygen in plasma (0.003 mL O2 per mmHg PaO2 per dL blood). Dissolved oxygen is a minor contributor at normal PaO2 but becomes significant at very high PaO2 (hyperbaric oxygen therapy).

Oxygen delivery (DO2) = CaO2 × cardiac output × 10. A patient with anaemia may have normal PaO2 and saturation but severely reduced CaO2 and DO2 — which is why haemoglobin is as important as pulse oximetry in anaemic patients.

Why does anaemia reduce oxygen delivery more than low saturation?+
The haemoglobin-bound component dominates CaO2. A dog with Hb of 7 g/dL at 100% saturation has a CaO2 of about 9.4 mL/dL — far less than a dog with Hb 15 g/dL at 95% saturation (19.4 mL/dL).
What is the oxygen delivery (DO2) formula?+
DO2 = CaO2 × cardiac output (L/min) × 10. Normal DO2 in dogs is approximately 500-1000 mL O2/min.
When is CaO2 clinically most useful?+
In anaemic patients, where saturation may appear adequate but actual oxygen delivery is severely compromised. Also in shock assessment and ventilator management.
What happens when CaO2 falls critically low?+
When DO2 falls below a critical threshold (critical DO2), tissues switch to anaerobic metabolism, lactic acid accumulates, and oxygen debt develops — a sign of inadequate oxygen delivery.
