How It Works

MVD progresses through ACVIM consensus stages: Stage A (at-risk breed, no murmur), Stage B1 (murmur, no cardiac remodelling), Stage B2 (murmur with cardiac enlargement — requires Pimobendan per EPIC trial), Stage C (active heart failure — medical management required), Stage D (refractory heart failure — advanced therapy). Median survival from diagnosis of Stage C (first heart failure episode) is approximately 9–12 months without treatment and significantly longer with appropriate medical management. The EPIC trial demonstrated Pimobendan delays onset of heart failure from Stage B2 by a median of 15 months.

What is the life expectancy of a dog with mitral valve disease?+
It depends critically on the stage. Dogs in Stage B1 often live years without progressing to heart failure — many remain stable for their entire remaining life. Stage B2 dogs treated with Pimobendan (per the EPIC trial) have a median 15+ months before heart failure. Stage C dogs treated with full medical management (Pimobendan, ACE inhibitor, furosemide) have a median survival of around 9–12+ months from first episode, with some dogs achieving much longer stable periods.
What is the EPIC trial and why does it matter?+
The EPIC (Effect of Pimobendan In dogs with preclinical mitral valve disease) trial, published in JVIM 2016, demonstrated that starting Pimobendan (Vetmedin) in dogs with Stage B2 MVD (confirmed cardiac enlargement) delayed the onset of heart failure by a median of 15 months compared to placebo. This was a landmark finding that changed the standard of care — Stage B2 dogs should receive Pimobendan. Stage B1 dogs do not benefit and should not receive it.
