What Is the Dog Stage B2 Heart Disease Life Expectancy Calculator?

Estimate life expectancy and time to heart failure for dogs diagnosed with Stage B2 mitral valve disease — based on EPIC trial data and ACVIM guidelines.

Stage B2 mitral valve disease is a critical juncture — your dog has a confirmed heart murmur AND echocardiographic evidence of cardiac enlargement, but is not yet showing signs of heart failure. The landmark EPIC trial proved that starting Pimobendan at this stage delays the onset of heart failure by a median of 15 months. This calculator gives Stage B2 owners specific prognostic estimates and treatment guidance.

Every result is based on published veterinary research and breed-size-specific growth data — not generic averages. This tool is completely free, works instantly in your browser, and requires no account or sign-up.

How It Works

Stage B2 MVD is defined by the ACVIM consensus (Keene et al. 2019) as: audible murmur, radiographic or echocardiographic evidence of left atrial enlargement (LA:Ao ratio ≥1.6) and/or left ventricular enlargement (normalised LV internal diameter in diastole >1.7). The EPIC trial showed: median time to heart failure was 15.0 months in the Pimobendan group vs 9.3 months in placebo — a 15-month delay. Dogs with more severe echocardiographic changes tend to progress faster. Cavalier King Charles Spaniels progress more rapidly than other breeds.

Frequently Asked Questions

What does a Stage B2 diagnosis mean for my dog?+
Stage B2 means your dog has a heart murmur caused by mitral valve disease AND echocardiographic evidence that the heart has started to enlarge in response — but has not yet gone into heart failure. This is a critical but manageable stage. The EPIC trial proved that starting Pimobendan (Vetmedin) at Stage B2 delays heart failure by a median of 15 months. Your dog may live for years without experiencing heart failure with appropriate management.
Should my B2 dog be on Pimobendan?+
According to the ACVIM 2019 consensus guidelines, YES — Stage B2 dogs meeting the EPIC criteria should receive Pimobendan. The EPIC trial is one of the most robust clinical trials in veterinary medicine, demonstrating a statistically significant 15-month delay to heart failure. If your dog has been diagnosed with B2 and is not on Pimobendan, discuss this with your vet or a veterinary cardiologist.
