Only 1/3 to 1/2 of the calculated deficit should be administered in the first 15-30 minutes — then blood gas reassessment guides further supplementation.

Bicarbonate therapy is reserved for severe metabolic acidosis (pH below 7.1, HCO₃ below 10 mEq/L) not responsive to fluid resuscitation alone. Target HCO₃ should not exceed 20 mEq/L in a single correction — over-correction causes metabolic alkalosis and paradoxical CSF acidosis.
