How It Works
Total calcium measured by standard laboratory tests includes protein-bound calcium (inactive), complexed calcium, and ionised calcium (the biologically active fraction). In hypoalbuminaemia — common in sick dogs — total calcium appears falsely low because the protein-bound fraction is reduced, even though ionised calcium may be normal. The corrected calcium formula adjusts for albumin: Corrected Ca = Measured Ca − Albumin + 3.5 (in mg/dL) or the equivalent SI formula. Alternatively, direct measurement of ionised calcium (iCa) on a blood gas analyser provides the most accurate assessment and is preferred in clinical practice when available.
Normal total calcium in dogs: 9.0–11.5 mg/dL (2.25–2.88 mmol/L). Normal ionised calcium: 1.25–1.45 mmol/L.
