How It Works

The VHS method (Buchanan & Bücheler 1995) measures the long axis (L) from the ventral border of the left mainstem bronchus to the cardiac apex, and the short axis (S) at right angles to L at the widest point. Both measurements are transferred to the vertebral column starting at T4 and expressed as the sum in vertebral body units. Normal for most breeds is 8.5–10.5v. Certain breeds have higher normal VHS — Labrador Retrievers, Cavalier King Charles Spaniels, Boxers, and many others have published breed-specific norms.

What is a normal VHS for dogs?+
The original Buchanan and Bücheler normal range is 8.5–10.5 vertebral body units (v) for most breeds. However significant breed variation exists — Labrador Retrievers have a published normal upper limit of 11.5v, Cavalier King Charles Spaniels 11.7v, and Boxers 11.6v. Always use the published breed-specific normal range when available, as using the generic normal will falsely identify cardiomegaly in breeds with naturally larger hearts.
What does an elevated VHS mean?+
An elevated VHS above the breed-specific normal indicates cardiac enlargement (cardiomegaly). This is used as part of the criteria for ACVIM Stage B2 mitral valve disease diagnosis (along with echocardiographic measurements). VHS alone is not diagnostic — it must be interpreted alongside physical examination findings, murmur grade, and echocardiography for definitive staging.
