How It Works

Glomerular filtration rate (GFR) is the gold standard measure of kidney function — it quantifies how much blood the kidneys filter per minute. In dogs, GFR is estimated from serum creatinine and body weight using the Modified Cockcroft-Gault formula adapted for canine physiology. A normal GFR in dogs is approximately 2–5 ml/kg/min. Values below 2 ml/kg/min indicate significant reduction in kidney function. SDMA (symmetric dimethylarginine) is now preferred as an earlier biomarker of GFR decline, detectable when GFR has fallen by approximately 40% — compared to creatinine which only rises when GFR has fallen by 75% or more.

The IRIS (International Renal Interest Society) staging system uses creatinine and SDMA levels to stage CKD into four grades, each with specific phosphorus restriction and treatment targets.
